Prize Details, Agreement & Regulations

Note: Before purchasing a ticket for this contest please read the following details, agreement and regulations, because once ticket/s are purchased it serves as your
willful agreement to adhere to everything written below.

The winner of the TalkHair.com Healthy Hair Model Contest will receive one of the following 12 month plans. The first winning ticket will be drawn on April 15, 2010 @
7:00pm and each month for the next 11 months, on the same day and time; the 15th @ 7:00pm. Each month starts a new contest cycle and new tickets must be purchased to enter the next contest on the following month and no one can purchase more than 5 tickets each month.

Note: all tickets must be filled out with complete and accurate information: Full Name, Address, Cell Phone, E-mail.

All tickets are $10.00 each; limit 5 per person per monthly drawing. All ticket sales are final and absolutely NO refunds, exchanges or credits.

Discover). Yes tickets can be purchased over the phone and the ticket will be properly filled out, placed in drawing container and ticket stub will be mailed to address
provided. No tickets can be purchased on-line. A portion of proceeds will be donated to CHKD and the Breast Cancer Foundation.

Plan A:
• 8 Relaxer services (based on a 6 week schedule)
• 12 Halo-relaxer services (relaxing only the edges and perimeter area of head)
• 8 Proplex Reconstructor Treatment Services (based on a 6 week schedule)
• 40 Shampoo and Style services of your choice
• 8 Permanent color services (Only if desired)
• 10 Semi-permanent/ rinse color services (Only if desired)
• 10-12 Hair Cuts of choice (includes trim)
• A Free Hair and Health Care Consultation twice a month

Plan B:
Hair Enhancement Clients
• 12 Bonding Services including hard-wraps, but excluding Lace front: based on a 4 week schedule (you must provide your own Hair and hair selection
must be agreed upon with Hair Stylist) or 6 Sew-in Services: based on an 8 week schedule (you must provide your own Hair and hair selection must be
agreed upon with Hair Stylist).

• 6 Relaxer services (based on a 8 week schedule)
• 12 Halo-relaxer services (relaxing only the edges and perimeter area of head)
• 8 Proplex Reconstructor Treatment Services
• 26 Shampoo and Style services
• 20 Semi-permanent/rinse color services (includes edges and/or whole head)
• 20 Hair Cuts of choice (includes trim)
• A Free Hair and Health Care Consultation twice a month

Before agreeing to either of the above mentioned plans please read the following:

  • Each of the 12 winners must agree to 2 photo shoots and sign a TalkHair.com Model release form and all Photographers release forms (all photos will be used for TalkHair.com calendars and other associated advertisements). All participants must be 18 years old and older or must have a parent or guardian sign all release forms mentioned above. Please contact Levon Holman @ (757) 639-2055 to sign release forms.
  • The winner acknowledges that all services within the plan must be used monthly, unless the winner of this contest becomes ill, hospitalized or deployed then their plan will be put on hold until they can continue and the Hairstylist assigned to service them will keep a detail record of services rendered as well as remaining services within their plan. Once the winning returns their plan will resume until fulfilled. The winner must adhere to detailed schedule of appointments agreed upon by the assigned Hairstylist and themselves. All services will have to used (except for undesired services) during the 12 month plan and there will be no services rolled over to the 13th month and beyond. Note: Undesired services are services that are in the plan as extras that a client would never desire during the whole 12 months. The winner acknowledges that any and all “No Shows” and “Canceled Services” will not be able to recover that service. A “No Shows” is any appointment that was scheduled with your assigned stylist that you did not call to cancel within a 24 hour period for regular services and a 48 hour period for Hair replacement services. “Canceled Service” is a service you had to cancel for any and every reason even if done before the 24 – 48 hour periods.
  • Each person that purchases a ticket for this contest fully acknowledges that the plan of their choice can only be used by them and them alone. No one
    else will be allowed to be serviced under either of these plans. However, if the winner of this contest becomes ill, hospitalized or deployed then their plan will be put on hold until they can resume their services. The Hairstylist assigned to service them will keep a detail record of services rendered as well as remaining services within their plan. Once the winning returns their plan will resume until fulfilled.
  • No winner can win twice within the 12 month contest period and TalkHair.com Management reserves all rights to choose which contest winner will be displayed on each month of the TalkHair.com Healthy Hair Calendar (no exceptions to this rule). All winners will be notified by phone and e-mail and scheduling of photo shoots and Hair service appointments will begin immediately thereafter.
